Stephanie Smith-Fornshell (She/Her/Hers) is a board certified, Psychiatric Mental Health Nurse practitioner who has interest in the whole person. She has a decade of experience as a provider in psychiatry treating individuals with mental illness, cognitive disorders, substance abuse, and dementia.
Stephanie began her nursing degree at age 16 as a CNA. She has experienced working a women’s healthcare setting. Stephanie is also a college didactic instructor at Walden University. Stephanie has one daughter and has four grandchildren who are her life. Stephanie spends time as an active member in her community as well as a mental health advocate.
Stephanie provides psychiatric assessments and prescribes medications for adults, ages 18+, and offers appointments via telehealth.
Education: BSN, The College of St. Scholastica, MSN, University of Cincinnati, and DNP with honors, Walden University.
Member of AANP, Sigma Theta Tau International Honor Society, and has been inducted to the National Honor Society of Leadership and Success
